Open your document in Google Docs.
In the top left, click
Insert
and then
Headers & footers
.
Choose
Header
.
Enter text for the header.
Use
Options
for entering page numbers.
Options > Header format - Allows you to create a different first page.
Options > Page numbers - Allows you to specify when page numbering should begin.
